Insurance Claim for Injury

HI,

There has been any experience filing an insurance claim for a person injury at a ward activity? We had a dance last Friday and our Primary President tripped over a bench in our cultural hall and dislocated her shoulder. So far their have been two trips to the ER, and a follow up doctors appointment yet to come, and the family in uninsured at the moment because of lack of employment issues.
I'm just information gathering here before I contact Deseret Mutual. I know a claim form needs to be filled out , although I have yet to locate that form on either the church website or Deseret Mutual's (you'd think they didn't want people to file a claim or something).

Any help will be apprecited.
